The radiation is not at the maximum power rating of the transmitter.

The radiation is not constant, it's pulsed. I don't know the approximate duty cycle, but roughly speaking it's over 100 (and actually more), otherwise the phones would interfere with each other, they work on the same frequency.

The power drops inversely proportional to the square of the distance to the antenna. That is, very, very fast.

An outboard antenna tells you nothing about the power.
