IMHO, the article is outdated in many respects.
There is no such form anymore, there is no possibility to specify a second phone number.

Hi Wesley.
In On Init the EA should be returning INIT Failed.
Is your EA designed to trade forex? See that it's trying to run on EURUSD (it could be another asset, validation is random).
I suggest you run a backtest of the EA on the same pair and analyse where the error occurred, correct it and try sending it again.

I developed an EA just to run on mini index and mini dollar (Bovespa).
But when it comes to validation, the system tests with Forex (EURUSD). So, obviously, no trade is executed and the validation test fails.
