Execute orders through CSV file with defined values for contract size, pending order "sell stop/"buy stop", defined ticks from actual price

Hello,

I would like to create a csv file with following values and need a skilled programmer:

What I expect: 

I will create this CSV-file and put this into a folder in MT4. As soon as I want to execute these orders (instantly) I will have to press a button or execute the script in experts.

It should be possible to choose all FX currencies in MT4. The distance from current price, S/L, T/P is defined in ticks , not pips. Slippage should not be considered.
