MA Envelope Indicator

I am looking for a moving average envelope for MT4. One that I can adjust by percentage a top band and a lower band and set the MA to a particular number of periods as well as having the MA show up in on the chart inbetween the upper and lower bands. I have attached a screen shot of an example that has been taken from VT Trader. Any help would be appreciated.

That 'screen shot' seems to be the write-only version.

 
Ok now I'm unsure whether I'm seeing attachments or not... Brewmanz made a reply as if he saw the SS, and yet i see no link or screenshot anywhere... has it been removed?
 
I never saw any screen shot or other attachment. I use the term 'write only' as a programmer's joke about something that cannot be read/seen (in contrast to 'read only' which cannot be written).
