CREATE SIMPLE SET PENDING ORDERS SCRIPT

I'd like to make PENDING ORDERS SCRIPT. 

I have got a sample in attachment, I just need something different in settings. 

Here's what I want to see in input settings:

Type: SELL LIMIT / SELL STOP / BUY LIMIT / BUY STOP (drop down menu)
First Order Price: 1.0
Fixed Lot Size: 0.01
Stop Loss: 20 pips
Take Profit: 12.5 pips
Number Of Orders: 3
Pips Between Order: 10


Note: my broker is ICMarkets, so I guess it's 5 Digits.
