I need a dialog with approximately the following design:



Requirements:

- should not import dll's

- the checkboxes (and inputs) to the right (on touch, ...etc) should all start in a disabled (greyed out) state

- the disabled checkboxes should only be enabled after the left most checkbox is selected

- the code does not need to be cross platform (I'd like an MT4 version and an MT5 version)

- the input boxes should allow decimal values (Ex: 0.01)

Improvements or suggestions are welcome.
