Indicator license protection

Hi, I have attached an image of what I'm looking for ideally. I have 4 indicators, and all have account number protection, and expiry protection. I'm currently looking for something as shown in the attached image to make it easier for me to distribute to clients. I need this doing on a fairly quick turnaround. If you are able to do this for me, please apply!
