ADX Alert at specified level - MT5 Indicator alert with push and email notifications

I'm looking for an ADX Alert with the following specifications:

- ADX Period (default 14)

- ADX Level (default 30)

- Pop-up Alert with sound (true or false)

- E-mail notification (true or false)

- PUSH notification (true or false)

Additional info:

- I just want one alert when ADX crosses above the specified level (just one alert when it crosses, not in every bar after that).

- I don't want an alert when it crosses back down, only when it crosses back up again.

- Standard MT5 ADX calculation (not wilders or smoothed etc)

- DI+ and DI- lines have no relevance... I don't wan't DI cross alerts!

- Want alert message format something close to '(CHART NAME) (CHART TF) ADX level crossed at (TIME)'

- MT5 indicator NOT MT4

- Developer should provide codebase
