Unfortunately, at times spreads can widen and excessively at times to 100 or 150 points
This is probably what happened in your case. The spread widened, remember that a Buy is settles at Bid and a Sell at Ask.
Take loss and stop profit lol ... I've written EA's that excel at doing that
Hi Albert, did you see with your own eye how it happened? I wonder if price went through the stops letting the order open for several more seconds.. i. e. between the stop price and the close price there were a couple of ticks. If this was the case, then you should leave the broker.

Hi, I just experienced this on 13 Nov 2013 23:50 (UTC+2) with my local broker. Is there anyone had same problem? Please help.
Order No 4154814: Closing Price @1282.41 while my Stop Loss @1281.15. There are 126 pips gap and make me loss extra USD 1,512 (126 x 12 Lot).
Order No 4157657: At the same time with order no 4154814, Closing Price @1281.15 same with my Taking Profit @1281.15. There is no gap here, because if there is a gap, I will get extra profit USD 630 (126 x 5 Lot).
Order No 4157661: At the same time with order no 4154814 & 4157657, Buy Stop Price @1281.15 has actual price @1282.41. Again there are 126 pips gap and make loss extra USD 882 (126 x 7 Lot).
Order No 4156832: Order has been canceled.
So, in my case Stop Loss and Take Profit function has become TAKE LOSS and STOP PROFIT. What a mess!
Worse because I lost USD 2,394 if refer to closing price @1281.15 as order no 4157657, or I lost USD 630 if refer to closing price @1282.41 as order no 4154814 & 4157661.
