Orders timing out

I'm getting several "off quote" errors when my EA tries to open trades. The orders are sent with a 5 pip slip deviation. My application writes to a logfile with the new, refreshed price, so I know the price has not moved much. It doesn't happen all the time, but I have several of these errors.

My broker says that the trades are timing out at their end after 180 seconds. Any ideas what could be causing this?
 
"Off quotes" means broker don't give quotes on your request. Ask question your broker
 
I have asked. They say the orders are timing out. What would cause that?
