Online account_number validation in PHP

Phase 1 of the project. 

I need a simple account number validation in PHP language. 

You can check the simple web layout in the attached image. 

No other features. just input account number and add button. 

I need the PHP files and EA Code structure and EA Oninit code. 

install to my domain to make sure that the code is working.
