Need a simple panel or dialog box that just has four buttons

Looking for someone to program a simple GUI panel or dialog box that just has a few object within it.

Requirements:

1. Main dialog box or panel.  Square or rectangle, size of the box is not important.
   
2. the main panel or dialog box is movable.  I would prefer by double clicking the mouse on the dialog box and dragging the dialog box. but this way is not required.
   
3. the dialog box will be able to be locked so it is unable to be moved by the mouse, but any button inside will still be functional.  I would prefer if it is locked by double clicking the dialog box again, but again this is not required.
   
4. Area at the top for a title label.  title can just be labelled "title".
   
5. four functional buttons inside the main dialog box/panel labelled: 15, H1, H4, D1 anchored to the dialog box/panel so if the dialog box moves the buttons move with it.
   
6. when single mouse clicking(selecting) each button it will change time frames.  --- button 15 to 15 minute chart, button H1 to hour 1 chart, button H4 to the hour 4 chart, button D1 to the daily chart---
   
7. the dialog box will remain where it is located if the chart changes symbols or time frames.  I don't want it to reset to the default location if I change time frames or charts.
8. I would like access to the source code as well.
9. The colours of everything are not important.  They can be any colours, just as long as the title and buttons are visible.
