If you find anything missing I will be more than happy to help and improve the current documentation to become more clear. To help you answer your questions: 

1) The blue windows are the time frame that ICT mentions in his strategy (check the official video on https://youtu.be/tRq1hyGGtl4), those windows are from  3 AM to 4 AM, 10AM to 11AM, and 2PM to 3PM New York time. Those are the time window for which you identify a possible trade using the silver bullet strategy.

If your broker time is not New York time, it is normal that you will not see them in the same time frame, please make sure to fill correctly the "Current Broker GMT Shift" and "Current NY to GMT Shift" inputs.

To facilitate this, let me copy the example I had put in the blog: 

 Let us take an example of the first window mentioned by ICT, which is from 3AM to 4AM New York Time. If my broker is hosted in Europe as example, most probably the broker time will GMT+3, and New York (during light saving is -4GMT). this means the first window will take place on my broker time between 10 and 11 AM.

• 10 AM = 3AM + 4 hours to get to GMT + 3 hours to get to my broker time.
• 11 AM = 4AM + 4 hours to get to GMT + 3 hours to get to my broker time.

There is no difference, you should be using a previous version. This was unified in version 1.1 and above, please update to the latest version.

all sorted out I use V1.3. Coder very supportive and reacted quickly on reported issues. After 1 day tested on tester I purchased it, own it 2 days this indicator helped me avoid 3 bad trades and won 1 trade already. recommend it as essential tool in scalping and intraday trading

The default parameters should fit the original strategy of ICT, if your broker time is at GMT+3.

Using another sessions indicator to give me asian high and low for now until I can get this one to show the lines for me. I put different colors since I use a dark chart but it only shows london high and low now the asian one.

Thanks for your comment and purchase.

The indicator, once loaded, will show the current active session/s high and low. 

Once kept on the charts, it will start a rollover and show the sessions high and low to keep the three sessions high and low.

If I am correct, your need will be to see all three sessions high and low, whether active or not, on the indicator loading to chart, I am right? 

If so will release this in the next release this weekend.

Yes I'd like to keep the current sessions up for the day every day. They make break levels to short, long from according to ICT so even though this is a silver bullet indicator I'd love that feature since it's part of ICT anyways. I'd appreciate it! Also allow to put a background between the lines as well so it looks like I have it on the screenshot would be a nice feature to make it more visual.

On the next update can you please add an option to disable alerts for the FVG? It seems super annoying to get alert for every FVG created. I sometimes have 5 charts open with this indicator and it spamming me alerts every few minutes it's becoming too much. I would like an option to only send me an alert when the Silver Bullet starts as far as the FVG I don't need alert for those. So an option to remove alert would be nice and alert for silver bullet begins would be a good feature.

So far loving the indicator thanks a lot for all your hard work well worth the money. I use it daily and all the prev day lines, etc play so well for liquidity sweeps it's nice to have all that in one indicator. Every update should be amazing for now on. Keep up the hard work!

And I agree with you a lot of alerts is annoying :), In the current version you can disable the alerts for the FVG inside the silver bullet window, just switch the flags below to false. I will add your suggestion to send alerts on the beginning of the Silver Bullet window too, very good suggestion as I personally sometimes miss some window :D I did it yesterday on GBPUSD.
