Hi, sorry for the delay - I've answered your questions below

1) The pop up alert window in MT5 references the chart and the timeframe which the alert was triggered from. Please see screenshot below

2) You will need to set up your MT5 account properly for push notification - you need to provide your MT5 client account with the device ID of your MT5 app (presumably on your phone or tablet).. then you're all set.

3) You can control the alert frequency and using the Maximum alerts parameter and also the Mininum Alert Interval parameter which controls the minimum time between alerts. The default setting will refresh the alert counter every minute so you'll need to configure the system to meet/match your requirements / expectations. 

If you host the indicator on a VPS you'll have to use push notifications to send alerts to your mobile MT5 app. You need to provice a medium for the VPS to send alerts to which is why it only works when you opened your MT app. If your VPS instance is configured to send email alerts then you could receive these on your mobile without requiring a local metatrader app to be open.
