Send Open Positions Details and Balance to MySQL db

Hello.   

I need an EA/script to send the Open Position details from an instance of MT4 to a table in a MySQL db, and the Account Balance to a separate table.  

For the Open Positions, the format should be:

Account | Symbol | Amount | P_L

account: the mt4 account number
symbol: position symbol
amount: lot size (sell trades should be expressed as a negative value)
P_L: the floating profit/loss of the position. 

For the Balance info, the format should be:

Account | USD | Balance 

(we're hard-coding sending USD)

There should be parameters for 
a.  MySQL connection (host, username, password, database)   
b.  Refresh frequency - how often to write the information to the db (in seconds)

I need this asap.   Please submit the cost, delivery time, and approach.
