"MT4Report.exe". 
Does anyone have a copy and would be so kind to share it?

There's a new version which can be download from the author's site here : MT4TOOLS
The registration cost is a real bargain for now at $9.95 for a full license or $4.95 for 6 months.
It will be the best $10 spent, trust me, this tool is a must for everyone dealing with big volume of results.
