I need some code to send and receive data from SQL server

Hi,

I need some code to insert and retrieve data from a simple SQL table. The table have only 5 fields. 

I know MQL4 but don't know any thing about PHP and SQL server. 

You should make me a script that can do the job then I implement it to my code on myself.
