Send Orders Info+Screenshot of Chart from Metatrader 4 to Discord

I need a simple solution for MT4 sending my trade info to Discord as signals

The EA will be attached to one chart on my platform, once there is a trade taken manually or from any EA you need to send trade info and current chart to the Discord

these is the info it should send:

Name: Trade Signal
Symbol: AUDUSD
Type: Buy 👆
Price: 0.72776
StopLoss: 0.71462 (70.00 pips)
TakeProfit: 0. 73776 (100 pips)
RR: 1.40
Time: 2020.08.10 17:33
Recommended Risk 1 % per trade

Once a trade is being closed/modified another notification with chart should be also sent to the Discord
