Web Based Deposit And Withdraw System for (Admin)

Dear Freelance,

We are Forex Broker and we have Metatrader4 Admin , we need a web based Deposit/Withdrawal form  with preferred PHP Language, with very simple template to just enter a Metatrader Account Number and Deposit/Withdrawal amount and proceed.

we need this with a full source code so our developer can merge with our admin CRM.
