Need A Horizontal Line Indicator / Script

I need a horizontal line indicator or script for plotting various levels with various types of lines.

I need it to have fields to enter the following:

Long Entry (here is a field to enter a price and at that price a solid green line is formed)

Stop Loss ( here is a field to enter a price and at that price a dotted orange line is formed)

Profit Target (this field needs to be able to have multiple entries and at that price(s) a green dashed line is formed)

It also needs a single field to give a label to each level. So if I put in the field "1" each line will be labeled "1")

However, I need the profits target field to give each entry a sequential letter. So if U put in 4 prices... the first price line is labeled 1a, the second is 1b, the third is 1c and the fourth would be 1d

I then need the following:

The ability to do the same for going short:
