Necessito inicialmente um robo que tenha uma tela de Input de Dados para análise, tal como coloquei no arquivo em anexo.

Em um primeiro momento necessito que gere um relatório (Output de Dados - planilha em anexo) que gere dados de prefere em Excel ou arquivo que possa importar para o Excel.
